This notice describes how personal member information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

How is Member Privacy Protected?

At Mended Hearts, we understand that information about you and your health is personal. Because of this, we strive to maintain the confidentiality of your personal information. We continuously seek to safeguard that information through administrative, physical and technical means, and otherwise abide by applicable federal and state guidelines.

How do we use and disclose personal information?

When you join Mended Hearts, we use and disclose your personal information for the normal organizational activities falling in the categories of dues payment and membership operations. Below we provide examples of those activities, although not every use or disclosure within each category is listed:

Dues Payment – We collect and maintain personal information about the status of your dues to the national association, Mended Hearts, Inc., as well as the local chapter, Virginia Peninsula Chapter 119. This information is shared with the national association to maintain your membership in good standing.

Membership Operations - We collect and use your personal information in the normal course of operating our non-profit organization. For example, as volunteers, the chapter relies on personal contact information to notify members of upcoming events, solicit volunteers for a fund raising event, and to send copies of our monthly newsletter, The Beat Goes On.

We may also use your health information to:

  • Comply with federal, state or local laws that require disclosure.
  • Respond to law enforcement officials or to judicial orders, subpoenas or other processes.

We may also use or disclose your personal or health information for the following operational purposes. We may: Contact you for Mended Hearts or American Heart Association fundraising.

Other uses and disclosures, not previously described, may only be done with your written authorization. You may revoke your authorization; however, this will not affect prior uses and disclosures.

 

What are Mended Hearts, Inc. Responsibilities?
